PALLIATION CHOICES
Palliation Care Team
• Nurse Practitioner will make initial consult, then
    follow up every 60 days
• More frequent visits made if medically necessary
    or if patient evaluation is needed per Island
    Home Care team
• NP services include, but not limited to: Evaluation
    and make recommendations for symptom
    management • Discuss care plans and
    treatment goals
• Skilled Nursing Services are available per
    admission criteria to Island Home Care
• Skilled Nursing Services include, but not limited
    to: Evaluation and care management of
    medical nursing needs of patient • Evaluation
    of prescribed medical treatment of pain and
    symptom control • Provision of emotional
    support to both patient and family • Help
    instruct caregivers in providing care to patient
• Social Worker to follow patient if consulted
• Social Worker services include, but limited to:
    Psychosocial assessment of patient and
    family • Coordination of community resources
• Physical Therapy and Occupational Therapy
    services available for consult per patient needs
• These services include, but not limited to: Improve
    quality of life through patient centered
    activities • Help provide pain/symptom relief
    (positioning, strength, stretching, etc) • Ease
    disease and treatment related fatique (exercise,
    establishment of daily routine, etc)
 
HOSPICE CARE
• Island Hospice (non-profit)
• Personalized care plans
• Nursing and therapy services
• Medical social work services
• Palliative care for pain and symptom management
• Bereavement and counseling services
• Chaplain services
• Music and pet therapy
• Trained volunteers (relief care, errands, etc)
• Medical supplies and medications related
      to diagnosis
