If your patient has an exacerbation of a preexisting condition, acute onset of a new diagnosis or new medication, home health services could be covered 100% by Medicare if the patient meets the eligibility requirements.

Home Care Services May Include:
- Observation and assessment
- Management and teaching
- Instruction on diet, disease process and medication
- Complicated wound management
- Lab work
- Administration of insulin, as ordered
- Teaching patient or family to mix and administer insulin
- Glucose checks, as ordered (call PCP if over or under range)
- Administration or teaching foot care regimen
- Teaching actions of ordered insulin
- Teaching urine check procedures
- Teaching symptoms of hyperglycemia and hypoglycemia
- Teaching emergency measures
- Nutritional support
- Patient education
